Care Management Coordinator

The Care Management Coordinator performs various duties supporting population health and clinical care management initiatives, including patient education, outreach, and referral management. They work with clinical providers and stakeholders to improve data collection methods and accuracy. The role also involves administrative tasks such as data entry, filing, and medical record management.

Requirements

• High School graduation or equivalent
• Current license as LPN in the State of Washington or current WA State Medical Assistant Certification
• 2 years of experience in health care, acute care hospital or ambulatory clinic
• Experience with electronic medical record (EMR) system
• Current Washington State Driver’s license and proof of insurability
• Reliable transportation
• Knowledge of medical terminology
• Experience with quality metrics to include HEDIS and STARS
• Ability to interact with others in a professional and courteous manner
• Excellent oral and written communication skills
• Exceptional organizational skills
• Proven ability to effectively and proactively complete multiple projects simultaneously and meet deadlines
• Strong attention to detail, accuracy, and ability to work independently
• Dependability and good judgement skills
