Job Description
EvergreenHealth is seeking a Physical Therapist Home Health to join their team. The Physical Therapist will be responsible for assessing, planning, implementing, evaluating, and providing skilled care to patients requiring physical therapy services in the home. The role involves providing educational information to patients and caregivers, communicating with other disciplines, and reporting changes in patient condition to the physician and case manager.
Requirements
- Current PT licensure to practice Physical Therapy in Washington State
- Recent applicable clinical experience in a medical/surgical unit, rehab, or home health setting
- Current Washington State Driver's license and proof of insurability
- Current Healthcare Provider BLS certification by date of hire
