Managed Care Organization Care Coordinator I

Coordinate follow-up visits and screenings for patients of all ages using telephonic outreach and email/text campaigns. Work with MCO Care Coordinators to submit supplemental data and close care gaps. Participate in report analysis and patient contact projects to improve patient healthcare.

Requirements

• Ability to function independently
• Flexibility to adapt to unforeseen needs
• Effective use of the Internet, Excel, and EHR systems
• 2 years' experience in a primary care setting
• Bilingual (Spanish/English) preferred
