Care Coordinator

The Care Coordinator coordinates patient care within the Health Home Program to ensure continuous and integrated care. Responsibilities include navigation of health, behavioral health, social systems, community resources, and housing transitions. The Care Coordinator works with patients, family members, and healthcare team members to reduce barriers to care.

Requirements

• Bachelor's degree in social work, nursing, health sciences, health education, or a related field
• At least one year of experience, preferred
• Ability to prepare complex documents in Microsoft Word and use Microsoft Excel and PowerPoint
• Good communication and interpersonal skills
