Job Description
We are seeking a Care Coordination Team Lead to provide field training and support for onboarding of care coordinators, assist with administrative tasks, and maintain an adjusted direct service caseload as assigned by the supervisor.
Requirements
- HSD/GED
- 2+ years experience working with individuals with primary mental health diagnosis and/or co-occurring disorders
- Valid driver's license and ability to maintain auto insurance
- CPR/First Aid Certification
- Ability to use computer apps including Microsoft Office, HRIS, EHR and other online systems
- Ability to travel between counties
