Job Description
Care Coordinator responsible for providing case management services to clients with primary mental health diagnosis and/or co-occurring disorders. Coordinates services, attends treatment plan development and review meetings, and provides support and resources for clients.
Requirements
- HSD/GED
- Valid driver's license and ability to maintain auto insurance
- CPR/First Aid Certification
- Ability to use computer apps including Microsoft Office and online systems
- Background check, drug screening, and fingerprinting
