Job Description
Join Family Support Services as a Health Care Coordinator and contribute to providing safety, stability, and quality of life for children and families. As a HCC, you will utilize a family-centered approach to assess, coordinate, and monitor client healthcare needs, and provide direct service to families in the community.
Requirements
- Certified Nursing Assistant (CNA), Licensed Practical Nurse (LPN), or Registered Nurse (RN) preferred
- Proven combination of education and experience demonstrating ability to lead, manage, enhance, and promote assigned programs
- Experience in home visiting preferred
