Job Description
Patient Service Coordinator is responsible for assisting patients and healthcare providers within the clinic, closing clinical gaps in care, and ensuring smooth transitions in care. Responsibilities include greeting patients, providing outreach services, rescheduling appointments, coordinating transportation, and communicating with medical staff.
Requirements
- Greet patients in a timely, cheerful, and respectful manner
- Provide outreach services and assist patients with completing forms
- Reschedule patients as needed and assist with referrals and external resources
- Coordinate patients' transportation
- Support clinical team to assist patients with medical questions
- Serve as a liaison between patients and medical staff
- Ensure timely delivery of outside mail and communication
- Cover front desk/call center vacancies as needed
