Job Description
The Referral Coordinator plays a vital role in connecting Forge Health’s/KPC's mission to the community by building partnerships and driving awareness of our behavioral health services across Long Island, NY.
Requirements
- 2+ years prior experience in behavioral health, Healthcare outreach, provider relations, or business development.
- Working knowledge of behavioral healthcare models, referral processes and provider workflows.
- Availability and flexibility to work outside standard 9–5 hours as needed to support referral/community partners
- Valid state license and access to a vehicle
