Job Description
Form Health is a virtual-only obesity medicine clinic and digital health company that provides physician-led obesity clinical care delivered by telehealth and covered by insurance on a nationwide scale. The Registered Dietitian is responsible for providing individualized medical nutrition therapy to patients through our telehealth platform.
Requirements
- Board certification in obesity highly preferred
- Positive, non-judgmental attitude
