Registered Dietitian - Diabetes Specialist

Form Health is seeking a Registered Dietitian to provide medical nutrition therapy to patients with diabetes and obesity via a telehealth platform. The ideal candidate has experience in diabetes and weight management, a positive attitude, and a passion for innovative care models.

Requirements

• Registered Dietitian with at least 3-5 years experience in counseling, behavior modification techniques, and working with patients with diabetes
• Certified Diabetes Care and Education Specialist (CDCES) highly preferred
• Experience working in a medical weight loss, with patients on weight loss medication, using meal replacement plans, leading support groups, and strong cognitive-behavioral therapy counseling techniques
