Patient Care Coordinator I

The Patient Care Coordinator I acts as a professional liaison for patients and families, care providers, and other Fred Hutch departments, providing continuity of care and developing a trusted relationship with patients to identify and assist with individual needs related to their care.

Requirements

• Schedules all patient appointments related to treatment within Fred Hutch and outside consults and referrals
• Coordinates scheduling with other departments to ensure efficient patient flow
• Updates patient and caregiver contact information in EHR
• Communicates with providers regarding scheduling needs, treatment schedule, and additional requests
