Isaacs Neighborhood Center are seeking a Case Manager to deliver effective case management services to promote optimal well-being and maintain stable housing for formerly homeless adults. The ideal candidate will have a Bachelor's degree and experience working with homeless adults and individuals with severe and persistent mental illness, substance abuse, and/or co-occurring disorders.
