Job Description
The Case Manager will deliver effective case management services to homeless adults at the Baxter Safe Haven, with the goal of securing permanent housing. The Case Manager will provide intake, assessment, service planning, advocacy, referral, and escort services to clients, as well as maintain case records and participate in staff meetings and training programs.
Requirements
- Associate degree required, Bachelor's degree preferred
- Experience working with homeless adults preferred
- Experience working with individuals diagnosed with severe and persistent mental illness, substance use and/co-occurring disorders preferred
- Prior case management in safe haven, outreach, or supportive housing experience preferred
- Bilingual (English and Spanish) preferred
