Admissions Nurse Liaison

Good Shepherd Community Hospice is seeking an Admissions Nurse Liaison to join its expanding Intake and Admissions team. The Admissions Nurse Liaison provides explanation of hospice services, conducts clinical assessments, and coordinates admissions.

Requirements

• Licensed by the Commonwealth of MA as a Registered Nurse
• At least two years of experience preferred in community health, oncology, hospice, or palliative care
• Highly skilled at providing patient assessments and evaluations
• Excellent communication and interpersonal skills
• Ability to work collaboratively with hospital staff, Good Shepherd colleagues, patients, and their families and referral sources
