Part - time Weekend Hospital and Admissions Nurse Liaison

Good Shepherd Community Care is seeking a 20-hour Weekend Hospital and Admissions Nurse Liaison to join its expanding Admission team. The role involves providing information about hospice services to patients, families, and hospital-based staff, conducting clinical assessments, coordinating hospital discharge plans, and maintaining professional relationships within hospitals.

Requirements

• Licensed by the Commonwealth of MA as a Registered Nurse
• At least two years experience preferred in community health, oncology, hospice, or palliative care
• Excellent communication and interpersonal skills
• Ability to work collaboratively with hospital staff, Good Shepherd colleagues, patients, and their families
• Highly skilled at providing patient assessments and evaluations
• Excellent problem-solving skills
• Competency with accepted standards of hospice nursing practice
