Job Description
Grace Community Care & Homes is seeking a Housing Case Manager/ Specialist to provide housing support services to individuals in the Bronx. The Housing Specialist will guide clients through the housing process and ensure sustainable housing outcomes.
Requirements
- Bachelor's degree in social work, human services, public health, or related field
- Minimum 2 years of experience in case management, housing navigation, or social services
- Knowledge of affordable and supportive housing systems is a plus
- Strong organizational, communication, and problem-solving skills
- Ability to manage a 30-45 client caseload in a field-based environment
