Job Description

Requirements
- Convey patient assessments with clinical data to hospice physician
- Provide direct patient care to terminally ill patients
- Develop, maintain, and manage an individualized care plan
- Document patient problems, nursing assessments, patient goals, care provided and patient/family outcomes
