Job Description
The Hospice RN Case Manager is responsible for coordinating and managing the care of patients in a hospice setting, providing skilled nursing services, and overseeing the implementation of care plans. Requirements
- Current Registered Nurse (RN) license in the state of practice
- Minimum of one year of experience as an RN in a hospice or related healthcare setting
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Valid driver's license and reliable transportation for home visits
- Current CPR certification
- Knowledge of regulatory requirements and guidelines governing hospice care and documentation
