Primary Care Physician (Summer 2027)

Greenbrook Medical is seeking a Primary Care Physician to provide high-touch, relationship-based primary care to seniors in the Greater Tampa Bay area. The ideal candidate will have 2-6 years of experience in a primary care setting and be certified in Internal Medicine, Family Medicine, Geriatrics or a similar specialty.

Requirements

• Provide daily care to patient panels capped at approximately 500 patients
• Deliver patient-centered care to geriatric patients within a managed care, value-based care model
• Manage chronic conditions, preventive care, and acute issues within the scope of value-based care principles
• Collaborate with your team and specialists to ensure seamless care delivery
• Achieve quality metrics by proactively addressing care gaps, reducing unnecessary hospitalizations, and improving patient engagement
