Home Health/Hospice Nurse (RN)

We are looking for a Home Health Nurse to join our small and specialized home care health team and provide exceptional care to our patients. Requirements

• Administer skilled nursing care to patients requiring intermittent professional nursing services in accordance with department policies and procedures.
• Complete initial evaluation visit, regularly re-evaluate the patients nursing needs, initiates the plan of care and furnishes services.
• Coordinates services works with physicians and other personnel of changes in patients condition counsels patient and families.
• Participates in in-service programs, and supervises and teaches other nursing personnel.
