Home Health/Hospice Nurse (RN), PT

Gunnison Valley Health is seeking a Home Health Nurse (RN) to join their small and specialized home care health team. The ideal candidate will have experience in wound care and a current RN license from the state of Colorado.

Requirements

• Administer skilled nursing care to patients requiring intermittent professional nursing services
• Complete initial evaluation visit, regularly re-evaluate the patients nursing needs, initiates the plan of care and furnishes services
• Coordinates services works with physicians and other personnel of changes in patients condition counsels patient and families
• Participates in in-service programs, and supervises and teaches other nursing personnel
