Case Manager - Brompton Heights

Brompton Heights is seeking a compassionate Case Manager to assess, plan, and coordinate case management activities for seniors in their Assisted Living, Enhanced Care, and Memory Care residents. The Case Manager will work with residents, families, and staff to ensure overall health and wellness.

Requirements

• Associate degree
• Three (3) years of prior experience in adult care facilities or long-term care buildings
• Prior supervisory experience
• Ability to maintain a flexible work schedule
• Detail oriented with strong communication, documentation, and computer skills
