ESD Community Health Coordinator

The Community Health Coordinator will report directly to the Enabling Services Manager and will be responsible for supporting outreach, health education, care coordination, and administrative functions to improve patient outcomes.

Requirements

• High School Diploma, or equivalent or AA degree
• Prior experience with at-risk families in health care or human services setting preferred.
• Bilingual English/Spanish required.
