Job Description
We are accepting resumes for per diem Behavioral Health Home (BHH) Wellness Coordinators to support clients in various counties. As the Wellness Coordinator, you will work alongside a team to assist clients manage and coordinate medical and mental health services.
Requirements
- 3 years of experience in social/healthcare services
- MHRT/C (Domains 1-3) certification to work with adults
- Bachelor's Degree in social services or related field (LSW preferred) to work with children
- Ability to work independently and as a member of a collaborative team
