Job Description
Provide medication management services to patients of Health Sciences North (HSN) including the evaluation of the appropriateness and effectiveness of patients’ medications, drug information, and interdisciplinary drug therapy management support and drug distribution services.
Requirements
- Minimum of a four (4) year Bachelor’s Degree in Pharmacy, from an accredited university.
- Doctorate of Pharmacy (PharmD) with a residency program recognized by the OCP is preferred.
- Current Certificate of Registration in good standing with the OCP.
- Three (3) years’ experience working in a community Pharmacy is preferred.
- Experience working in a clinical Oncology Pharmacy environment is preferred.
- Experience in academic teaching with health science learners is preferred.
