Job Description
Healthfirst is seeking a Hybrid Assessment RN to conduct home assessments and provide care management services to members. The role involves working from home part-time and conducting in-person assessments in the Bronx, NY area. The position requires a New York State RN license, ability to travel, and prior work experience with technological proficiency and Microsoft Office skills.
Requirements
- Conduct home assessments and virtual assessments as needed
- Navigate multiple technology platforms
- Complete assessments of service needs and determine necessary adjustments
- Liaison between members and care management teams
- Provide feedback to home care agencies and contract nurses and aides
- Ensure members understand and agree with enrollment in a managed long-term care plan
