Licensed Social Worker - Inpatient Hospice Discharge Coordinator

Requirements

• Master's degree in social work
• Licensure/ Registration/ Certification: Must have Minnesota (MN) Social Work license upon start date: Licensed Independent Social Worker (LISW) or Licensed Independent Clinical Social Worker (LICSW)
• Must have vehicle, driver's license and current auto insurance. Must meet all criteria required under Park Nicollet’s Automobile Liability Guidelines
