Licensed Social Worker - Inpatient Hospice Discharge Coordinator

We are looking for a Licensed Social Worker to join our Hospice team. The role involves providing supportive services to patients and their families, collaborating with the interdisciplinary team, and working with community resources.

Requirements

• Master's degree in social work
• Minnesota (MN) Social Work license upon start date
• Licensure: Licensed Independent Social Worker (LISW) or Licensed Independent Clinical Social Worker (LICSW)
• Ability to function independently with minimal supervision
• Computer experience including documentation in an electronic medical record
• Ability to effectively communicate in written and verbal form within the interdisciplinary team
