Job Description
The role of the hospice social worker is to deliver varied social work services to Hospice patients and their families, including emotional, spiritual, and psychosocial assessments, ongoing counseling, bereavement services, and community education. This position is part of the hospice interdisciplinary team and works approximately 24 hours per week.
Requirements
- Master’s degree in social work
- 1+ year of experience
- Current LGSW, LISW, or LICSW in the State of Minnesota
- Excellent organizational and problem-solving skills
- Ability to read and interpret documentation
- Basic computer skills including Excel, Outlook, Word, etc.
- Ability to manage stress & emotional challenges
- Ability to act and make decisions independently based on the values of Olivia Hospital & Clinic
- Strong critical thinking skills
- Effective communication skills
