Job Description
The hospice social worker is an integral part of the hospice interdisciplinary team. The role involves delivering varied social work services to Hospice patients and their families, including emotional, spiritual, and psychosocial assessments, counseling, bereavement services, and community education.
Requirements
- Master’s degree in social work
- 1+ year of experience
- Current LGSW, LISW, or LICSW in the State of Minnesota
- Excellent organizational and problem-solving skills
- Ability to read and interpret documentation
- Basic computer skills including Excel, Outlook, Word, etc.
- Ability to manage stress & emotional challenges
- Ability to act and make decisions independently based on the values of Olivia Hospital & Clinic
- Strong critical thinking skills
- Effective communication skills
