Clinical Case Manager

As a Clinical Case Manager, you will be responsible for delivering case management and crisis intervention services to residents in a supportive permanent housing program. The role involves assessing the program service level of tenants, promoting independent living skills, and increasing housing stability within contract parameters.

Requirements

• MSW or related field
• Minimum two (2) years’ experience in delivering clinical crisis services
• Experience in working with homeless populations, especially families, and the Housing First model preferred
• Knowledge of AWADS is a plus
• Computer-literate with Microsoft applications, required
• Valid NYS driver’s license is a plus
