Job Description
The Wraparound Facilitator will be responsible for arranging information collection, meeting with families, assessing risk, and providing in-home outreach case management.
Requirements
- Bachelor’s Degree in Social Work, Human Services or Psychology
- Two years full time work experience with high risk children/youth in a Wraparound program.
- Bilingual in English and Spanish
- Ability to work a flexible schedule, including some weekends and evenings.
- Valid California Driver’s license, current auto insurance and good driving record.
- Ability to pass background check and criminal clearance.
