Community Health Worker, Enhanced Care Management

The Community Health Worker will serve as a case manager on the Enhanced Care Management (ECM) Team, providing additional health and mental health resources, services, and long-term support to individuals in the ODR program. The CHW will work collaboratively with a multidisciplinary team to ensure clients' healthcare needs are met through enhanced services.

Requirements

• Minimum 2 years working with unhoused, Justice Involved or seriously mentally ill clients
• Experience providing care support, community health collaboration, and direct client support
• Ability to work in the field and respond to community-based crisis situations
• Strong communication, empathy, and relationship-building skills
• Valid California driver’s license and ability to safely transport clients, per program needs
