Requirements

• Support and manage clients’ care from start to finish
• Create and update care plans, conduct care assessments, and supervise Care Professionals
• Provide guidance to team members to ensure outstanding care delivery
• Complete quality assurance visits and maintain compliance and client satisfaction
• Attend care consultations, complete care plans, prepare client documents, and carry out Care Professional introductions
• Prepare for and conduct quality assurance and service review visits
• Identify opportunities for excellent care and improving outcomes
