Full Community Health Worker- Behavioral Health Home position at Honor Community Health, a 501c3 Federally Qualified Health Center providing comprehensive primary, behavioral health, and dental care to underserved populations in Oakland County. The CHW will work with patients and their representatives to deliver integrated care and assist with referrals to community and social support services.

Requirements

• High School Diploma or equivalent required
• Current MiCHWA Community Health Worker certification or ability to obtain within 12 months of hire
• Two years of experience directly related to duties and responsibilities specified or lived experience in the communities served by Honor Community Health
