RN Case Manager - Hospice

Hospice RN Case Manager position available in Arlington, Texas. Lead with compassion and take ownership of outcomes. Provide expert nursing care for home and facility-based hospice patients.

Requirements

• Current Texas RN License
• Valid Texas driver’s license, reliable transportation, and auto insurance
• Graduate of an accredited Diploma, Associate, or Baccalaureate nursing program
• 1+ years of nursing experience (hospice, palliative care, oncology, home health, or acute care preferred)
• Ability to work Monday–Friday (8:30 AM – 5:00 PM) with rotating weekend on-call availability (full-time)
• Strong time management, organization, and documentation skills
• Experience with EMR systems (Consolo EMR a plus)
