Hospice RN Case Manager

RN Case Managers at Trinity Hospice build meaningful relationships with patients and families, manage visits, and advocate for patients with autonomy and support from an elite clinical team.

Requirements

• Active RN license in the state you'll practice
• Graduate of an accredited nursing program (ADN or BSN)
• 2+ years of RN experience with hospice experience required
• Strong clinical judgment, compassion, and communication skills
• Reliable transportation, valid driver's license, auto insurance, and CPR certification
