Job Description
As a Hospice Registered Nurse Case Manager, you will serve as the primary nurse coordinating care for assigned patients, developing and implementing individualized plans of care, and providing guidance to Hospice Aides and LPNs. You will also communicate changes in condition to physicians and the care team, and participate in on-call rotation.
Requirements
- Active RN license in the state you'll practice
- Graduate of an accredited nursing program (ADN or BSN)
- 2+ years of RN experience preferred (hospice experience a plus—but not required)
- Strong clinical judgment, compassion, and communication skills
- Reliable transportation, valid driver's license, auto insurance, and CPR certification
