Job Description
We are seeking RNs to provide hospice care to patients in their homes in Charles County, Maryland. This is a full-time position with a 40-hour schedule, Monday-Friday. As a Case Manager, you will provide nursing care and case management to patients with life-limiting illnesses, working with an interdisciplinary team to provide physical, psychosocial, and spiritual care.
Requirements
- Current Maryland RN license or accepted reciprocal out-of-state nursing licensure
- BSN preferred
- 2 or more years of nursing experience in med/surg or home care required
- Experience in hospice, end-of-life, or critical care experience preferred
- Familiarity and comfort with documenting in an Electronic Medical Record
- Valid driver’s license with an acceptable driving record and reliable transportation
- Ability to meet the physical demands of the position with or without reasonable accommodation
