Job Description
Case Manager - Care Coordinator role provides comprehensive support services to families experiencing homelessness, conducting assessments, developing service plans, coordinating care, and connecting clients with resources. The role involves working collaboratively with Emergency Shelters clients to promote stability, self-sufficiency, and long-term housing solutions.
Requirements
- Bachelor’s degree in Social Services, Human Services, or a related field
- Demonstrated experience in case management or a related role within nonprofit settings, homeless services, mental health, or supportive housing programs
- Basic computer proficiency, including familiarity with Microsoft Office applications (Word, Excel, Outlook) and database systems such as HMIS
- Valid California driver’s license and a clean driving record
- Reliable personal vehicle with current proof of auto insurance
