Job Description
Housing Case Managers provide comprehensive case management services to individuals experiencing homelessness or at risk of homelessness, conducting assessments, implementing harm reduction strategies, and coordinating resources to empower clients to attain self-sufficiency and achieve housing stability.
Requirements
- Conduct assessments, implement harm reduction strategies, and deliver trauma-informed care
- Coordinate resources, including connecting clients with local community resources and referring to other agencies
- Assist with unit applications, provide transportation for clients to property viewings, and enroll families & individuals into the Homeless Management Information System through the Coordinated Entry System (CES)
