Housing Case Manager

Illumination Health + Home is a non-profit organization providing targeted services to disrupt the cycle of homelessness. Housing Case Managers provide comprehensive case management services to individuals experiencing homelessness or at risk of homelessness, conducting assessments, implementing harm reduction strategies, and coordinating resources to empower clients to attain self-sufficiency.

Requirements

• Maintain a caseload of 30-35 clients
• Establish and execute an individualized housing plan, tenant screening, and tenant assessments
• Engage with clients weekly, utilizing three different methods of communication
• Identify independent living skills or goals in client care plans and track milestones
• Connect clients with local community resources and offer referrals to other agencies for additional housing opportunities
• Attend Interdisciplinary Care Team meetings to evaluate client goals and track the completion of action items
