Job Description
Illumination Foundation is seeking a Lead Case Manager to assess the strengths and needs of homeless clients, develop care plans, and oversee client care within assigned programs. The ideal candidate will have 2+ years of experience in social services, case management, or a related field and a valid CA driver's license.
Requirements
- Assess client strengths and needs
- Develop care plans for clients
- Oversee client care within assigned programs
- Meet with clients as needed to carry out care plan goals
- Provide crisis intervention for clients
- Supervise assigned case managers and care teams
- Track client and program data using Google docs and HMIS
- Complete weekly and monthly reporting
- Maintain updated and accurate case notes
- Collaborate with community providers
- Demonstrate a commitment to Illumination Foundation's mission and core values
