Job Description
As an integral part of the overall care team, the RN (Registered Nurse) Care Coordinator will collaborate with other professionals to ensure the best possible support is provided to individuals with intellectual disabilities and/or a behavioral health diagnosis. This role will provide support in a hybrid environment, including in-person and virtual support.
Requirements

- Experience with physical health disorders, psychotropic medications, DSM-IV diagnoses, and co-occurring disorders
- RN license through an accredited program (Associate or Bachelor of Science degrees are accepted)
- At least three years of experience in psychiatric, emergency, or substance abuse treatment nursing
- Valid licensure in the state of Iowa as a Registered Nurse
- Driver's License
- Ability to lift up to 35 pounds, prolonged sitting and working at a computer, frequent bending, stooping, and stretching, prolonged standing
