Job Description
RN (Registered Nurse) Care Coordinator needed to empower individuals with intellectual disabilities and/or a behavioral health diagnosis. As an integral part of the overall care team, you will collaborate with other professionals to provide support and resources to individuals in crisis, develop and implement care plans, and advocate for individuals to live independently.
Requirements
- RN License
- Hybrid position - Travel Expected

- Experience in psychiatric, emergency, or substance abuse treatment nursing
- Valid licensure in the state of Iowa as a Registered Nurse
- Driver's License
