Job Description
Imagine is seeking a Registered Nurse (RN) Care Coordinator to join our team. The ideal candidate will possess a RN license, 3+ years of experience in psychiatric, emergency, or substance abuse treatment nursing, and excellent communication and problem-solving skills. The RN Care Coordinator will serve as a medical/behavioral representative on the Responsive Services Committee, provide oversight and support to individuals with complex medical and/or behavioral challenges, and assist in the development and implementation of individual care plans. This is a hybrid position with travel expected.
