We are looking for a per diem Peer Support Specialist to join our team in Sussex County, Delaware. The Peer Support Specialist plays a crucial role in the recovery process as individuals attempt to navigate behavioral health systems and recovery resources. The Specialist educates and empowers individuals by assisting them in developing a strong, stable recovery support system.
Requirements
- Arrange medical appointments, therapy sessions, support groups, and other resources to aid the client
- Monitor the resident's behavior, schedule, medication, and activities
- Report any unusual behavior, conflicts, or recovery deviations to the relevant professionals
- Liaise with family members and medical professionals regularly
- Provide culturally appropriate recovery education and support
- Refer and link clients to appropriate services, clinical and/or medical services as needed within the agency and in the community in an appropriate and timely manner
- Collaborate with the resident care team to develop a plan that is individualized for the resident
- Provide care in a non-judgmental, non-discriminatory manner that is sensitive to the resident’s diversity, preserving their autonomy, dignity and rights
