Job Description
Included Health is seeking a Newborn/Pediatric Nurse Care Manager to deliver integrated remote care for members through their healthcare journey. The role involves guiding members through complex medical and behavioral health situations, partnering with a clinical team to create cohesive care plans, and providing compassionate follow-up care. This position requires excellent communication, clinical skills, and a proactive approach to member needs.
Requirements
- Bachelor of Science in Nursing (BSN)
- Current CCM Certification or eligibility within 6 months of hire
- 5+ years of experience in nursing
- 2+ years experience working in care management
- Active Compact RN license with good standing
- Willingness to become and maintain licensure in multiple states
- Must reside in a compact NLC state
- Spanish speaking desired
- Experience working remotely
- Strong competence in communication and empathetic care
