Job Description
Included Health is seeking a Newborn/Pediatric Nurse Care Manager to lead a care and case management team, guiding members through complex healthcare scenarios. This role involves delivering remote care, creating care plans, advocating for members, and providing clinical support. The ideal candidate will be passionate about holistic care and possess clinical skills to navigate benefits and resources.
Requirements
- Bachelor of Science in Nursing (BSN)
- Current or eligibility for CCM Certification
- 5+ years of nursing experience
- 2+ years of experience in care management
- Must reside in a compact NLC state
- Active Compact RN license
- Willingness to become and maintain licensure in multiple states
- Ability to work 9AM-6PM PST
